DESCRIPTION:

This position involves the provision of an array of training, support, and IADL’s services with a minimum amount of psycho-educational interventions provided within the community setting. Crisis Stabilization Specialists are responsible for all client related documentation and reports as well as the development and implementation of treatment goals and objectives. Crisis Stabilization Specialists are professionally qualified and registered by the board to provide collaborative mental health services for adults. Crisis Stabilization Specialists shall not engage in independent or autonomous practice. Crisis Stabilization Specialists shall provide such services as an employee or independent contractor of the DBHDS, or a provider licensed by the DBHDS.

TASKS:

  • Provide face to face activities, instruction, interventions, and goal-directed training that are designed to restore functioning and that are defined in the ISP.

  • Maintain timely and accurate documentation of service provision.

  • Attend monthly staff meetings and monthly supervisions consistently.

  • Ability to maintain a caseload of 1 to 5 clients.

  • Maintain a high degree of client confidentiality.

  • Work with other staff assigned to cases, communicating effectively.

  • Maintain a high degree of professionalism in the community, with clients, and with referring agencies seeking to build and maintain positive relationships.

  • Assist in providing Crisis Stabilization sessions to clients in the home and community setting.

  • Knowledge of community resources and entitlement benefits.

  • Bilingual a Plus, but not required

MINIMUM QUALIFICATION: 

Applicant has to be a “Mental Health Professional” with a bachelor’s degree from an accredited college in an unrelated field; that includes at least 15 semester credits in a human services field. In addition, who has at least three years’ successful clinical work experience (providing direct services to individuals with a mental illness, receiving gerontology or special education services).  12 VAC 35-105-420.

DESCRIPTION:

Position is responsible for providing diagnostic initial assessments, comprehensive assessments, prior service authorization request, initial ISP’s, comprehensive ISP’s implementing treatment plans, and re-assessing clients’ needs for continued stay. Responsible for occasional clinician supervision over designated direct care staff.

TASKS:

  • Provides assessment, short-term treatment, and case management to client caseload.

JOB TITLE:

Program Manager 

 

IMMEDIATE SUPERVISOR:

DESCRIPTION:  Position is responsible in assisting the CEO in the day-to-day operations of the all the programs. The Program Manager is responsible for new employee orientation, ongoing supervision and training of community-based case managers. This position is responsible for the overall case management of clients, to include medical, counseling, educational, vocational, and recreational services. On call services is also required as needed. Duties include but are not limited to training, direct staff supervision, client counseling, and advocating for the clients, whenever necessary.  You must maintain a home telephone and a DMV license.

TASKS:

  • Assist with field supervision of Case Manager for clients, monitoring interaction and rapport.

  • Assist with the implementation of daily program structure to include treatment and therapy.

  • Ensure that Case Managers prepare ALL reports on client treatment progress

  • Act as contact person/case manager for all client-related needs

  • Provide overall supervision of Case Managers, ensuring the teachings of daily living skills and treatment, implementing the program structure

  • Assist Case Managers with providing individual and group counseling to clients

  • Develop treatment team to assist with monitoring client’s behavior

  • Review staff’s, documentation and monitoring of client status in daily counseling notes.

  • Submit client’s authorizations and regularly monitor the authorization dates

  • Guide case Managers with developing social and daily living skills for clients

  • Assist staff while role modeling and conducting regular group meetings

  • Coordinate, monitor and maintain general supplies, office supplies, personal items

  • Implement standards of ethics

  • Interviewing potential staff
